As a first step, the Cordata+Care Continuity strategy team is focusing on testing the offering of a deeper and more comprehensive care coordination solution, to get some immediate client feedback sessions. Care Continuity designs, implements, and manages care coordination programs for hospital systems and convening organizations. Their work at Cordata-client hospitals and systems will allow the joint strategy team to explore ways to quickly offer the Cordata+Care Continuity client, deeper coordination benefits to users on the ground caring for the most vulnerable patients.
